Our Urgent Response Program (UR) provides support, resources, and advocacy for individuals and their support networks in urgent situations to prevent the further escalation of risk, mitigate any untenable risks, and address an unmet urgent need.
Our Urgent Response Case Manager works collaboratively with community partner agencies, including DMHS/LHO as part of a multi-disciplinary team strategy to provide a temporary, short-term case management response.
UR (Urgent Response) is a time-limited, collaborative, and goal-oriented process delivered to access, navigate, plan, coordinate, and develop options for services and supports required to achieve temporary or long-term mitigation of unmet urgent needs.
Who is eligible?
To be eligible for the UR Program, individuals must be deemed eligible by Developmental Services Ontario (DSO) and be:
· 18 years or older
· Have a developmental disability and mental health problem, and/or challenging behaviour
· At risk of:
o caregiver burnout
o housing instability
o support breakdown due to increased support needs
o involvement with the criminal justice system

What is not part of Urgent Response?
· UR is not a crisis support service. If, as a result of the screening process, the family is determined to be in crisis, the UR Manager will help them connect with appropriate crisis services in the community
· UR does not operate on a 24/7 basis
· Receiving UR Support does not provide fast-track access to any service, including housing, within Developmental Services. Families will be referred to other services and will be required to follow their intake protocols and waitlists for service.
· UR is not an out-of-home residential service
· UR does not provide direct funding to purchase supports
